Chronic Disease Management Plan for Physiotherapy

Managing a long-term health condition can be physically and financially demanding.
If you are living with a persistent medical issue that your doctor is aware of, you may be eligible for a Chronic Disease Management (CDM) Plan—formerly known as an Enhanced Primary Care (EPC) plan or a Team Care Arrangement (TCA) – offered through Medicare.
This system is designed to help patients access essential allied health services, providing a practical way to receive reduced-cost, early physiotherapy care when you need it most.

How the System Works A CDM plan is initiated directly by your General Practitioner (GP). If your doctor determines that your condition meets the criteria for being chronic, they can formulate a management plan. As part of this process, your GP can prepare a referral that specifically allocates Medicare-subsidised visits to Physiotherapy.

At Physioform, we proudly accept these GP referrals, making it seamless for you to transition your care directly into our clinic.

Benefits for Your Long-Term Health

  • Reduced-Cost Early Intervention: The Medicare rebate offsets out-of-pocket expenses, making it easier to seek professional guidance early before a chronic issue escalates.
  • Collaborative Medical Care: Because your GP coordinates the plan, your physiotherapy treatment is closely tied to your broader healthcare team for cohesive management.
  • Structured Support: Allocated sessions provide a reliable bridge to help you establish consistent exercise habits, control pain, and build functional capacity.

If you believe you may qualify, schedule a conversation with your GP to discuss your eligibility.
Once your referral is secured, our team at Physioform is ready to help you put those subsidised sessions to work.
